How to use

  1. Audit ≥1×/year, covering all items below (= all 5 criteria).
  2. Auditor must be independent of the area audited.
  3. Every NC gets a row in the CAPA register.
  4. Results feed the management review.

Checklist (items by criterion)

Criterion 1 — Commitment and Responsibility (items 1–7)

#Checklist question
1Has the halal policy been established? (printed & displayed)
2Has the halal policy been socialized/disseminated? (documentation of socialization)
3Has a halal supervisor been appointed and updated per HR conditions? (appointment decree)
4Has the supervisor attended training / competency testing for halal supervisors? (certificate)
5Does the supervisor have a job description matching duties & responsibilities?
6Is the supervisor registered with BPJPH?
7Are SJPH training activities (internal/external) held ≥1×/year? (implementation documents)

Items 1–7 tested commitment; items 8–14 now test the material base.

Criterion 2 — Materials (items 8–14)

#Checklist question
8Do all materials use halal-certified materials or materials exempted from certification (positive list)? — raw materials; additives; processing aids; packaging, lubricants, grease, sanitizers; cleaning aids; media for validation of facility cleaning
9Do materials meet safety/health aspects per applicable regulations?
10Is a material list available?
11Are materials purchased/used with name/brand/manufacturer matching the BPJPH/LPH-approved material list?
12Are there material changes? Describe changes used.
13Are supporting material documents available (production process, composition, CoA/RDS) and valid?
14Are purchase records/receipts and label samples kept for the certificate validity period?

With materials verified, the checklist turns to the production process itself.

Criterion 3 — Process / PPH (items 15–18)

#Checklist question
15Are PPH locations, places and equipment separated for halal vs non-halal processes? (slaughtering, processing, storage, packaging, distribution, sales, serving)
16Are PPH locations, places and equipment clean, free from najis, free from contamination by non-halal materials?
17Is a layout/site plan of the PPH location available?
18Are equipment containing animal-derived elements used? If yes, is a halal certificate document available for them?

Process checks complete, the audit moves on to what the customer actually receives.

Criterion 4 — Products (items 19–26)

#Checklist question
19Are products produced per the product list in the certificate appendix? (product list uploaded)
20Are there additions/product developments not yet submitted / still applying? (list added products)
21Do products originate from halal materials and are processed per PPH and Islamic law?
22Do products meet safety/health aspects per applicable regulations?
23Do product names, forms and/or sensory characteristics contradict Islamic law, public ethics, or lead to haram products?
24Does packaging meet halal aspects? (matches contents; carcass packaging clean/odorless/safe; design complies)
25Is the halal label on packaging per regulations? (packaging design per certificate product list)
26Is traceability identifiable and does it guarantee the halalness chain (materials → PPH → product)?

Product checks close the front half; the monitoring questions below finish the loop.

Criterion 5 — Monitoring and Evaluation (items 27–32)

#Checklist question
27Is there a program for monitoring & evaluation: internal audits and management reviews periodically (≥1×/year)?
28Are documented procedures for internal audit and management review available?
29Has an internal audit been conducted ≥1×/year monitoring SJPH implementation?
30Has a management review been conducted ≥1×/year evaluating SJPH implementation?
31Have records of internal audit and management review been maintained?
32Does the company take corrective actions on internal-audit findings?
